To Schedule an Appointment:
A) Established patients can press the "purple calendar icon" from the "visits" tab in the Athena Patient App either on their phone or online OR

B) New or Established patients can click below to schedule

Patient Portal Information
You should have received an email from the office with sign up instructions on how to create your online patient portal account. If you have not, please do NOT create a "new account" online as this will cause duplicate charts and not allow you to log in, instead, call the office to have the link sent again to your email. To sign in to your Patient Portal, download the "Athena Patient" application from your phone app store or log into your account online by clicking below:
