To Schedule an Appointment:
We ask that you arrive at least 20 minutes before your appointment starts. ALL paperwork will need to be completely finished BEFORE your appointment start time or else we will need to reschedule your appointment.

You MUST have a photo ID (even if <18 years old) and your insurance card.

Patient Portal Information
You should have received an email from the office with sign up instructions on how to create your online patient portal account. If you have not, please do NOT create a "new account" online as this will cause duplicate charts and not allow you to log in. Instead, call the office and we will send the link to your email again.
